WHAT DATA IS BEING COLLECTED?
As part of visiting The Natural Therapy we will need to have a record of your personal details: date of birth, address, telephone numbers, email and relevant medical information relating to your session.
Personal data about your presenting symptoms and the therapy provided will also be documented in detail. You have the right to request a copy of your records.
Your session notes will be held in a locked cabinet drawer. No client files are accessible to anyone other than your practitioner. Your notes will be kept for 8 years after your last session because of data protection laws. For children, notes will be kept for 8 years past the age of 16. The notes will then be shredded and disposed of safely
For communication purposes your name and email address will also be stored in ‘Contacts’ in ‘Mail’. If an email is sent to more than one recipient, the email addresses will be entered in Bcc to ensure other recipients are not able to access your email address.
When an appointment is made your name will be entered in a paper diary and/or digital calendar, no other personal data is included.

WHO IS COLLECTING DATA?
Your practitioner is collecting data at the start of your first session. Some information may be requested by email, text message, telephone or post to ensure the smooth running of your therapy. On occasion data from relevant medical notes/letters and scans obtained, with your written consent, from other healthcare practitioners may also form part of the data collected and held by your practitioner.

HOW IS DATA COLLECTED?
Collection of data will happen via pen and paper, secure email, text messages, photographs, videos, and letters by mail. No personal data will be collected via social media.

WHY IS DATA BEING COLLECTED?
Data is collected to record, guide and supervise your progress and to be able to communicate effectively with you for the best outcomes. It is also used to compare progress week to week and to highlight changes, note any concerns and any action to be taken. It is also a detailed record of your therapy sessions.
Data also helps us to carry out relevant research from time to time. Your written permission will be required to use your data for these purposes.

HOW WILL YOUR DATA BE USED?
Data will be used to communicate appointments, record session information, document progress and to detail relevant referrals.
Data may also be used to send newsletters, promotional offers and healthcare information. You may opt out of these communications if you wish.

WHO WILL DATA BE SHARED WITH?
Data is rarely shared outside of the clinical environment. On occasion you may be asked for permission for the information to be shared with another practitioner or medical service for referred treatment or to report clinical outcomes to your GP.
Full written permission will be requested before your data is shared.
Client experiences (testimonials) may be shared with the public with full consent from the client. Consent will be given by a signed consent form prior to sharing.

WHAT WILL BE THE EFFECT OF THIS ON THE INDIVIDUALS CONCERNED?
Client’s data will be kept safe and in accordance with GDPR.
No data is shared with third parties without consented permission.
No data is sold to third parties.
No data is held on phones unless encrypted with a pin number/finger print recognition. No phones are left unattended.
All computers/laptops and tablets are locked with passcodes and are not left unattended. Only individuals with permission to read notes can access this data.
